Rural Health Care, Inc. d/b/a/ Aza Health is a private non-profit community and migrant health center established in rural Putnam County, Florida in 1977. A federally qualified health center (FQHC), Aza Health operates in areas designated as Health Professional Shortage Areas and or Medically Underserved Areas or Populations.
Most Aza Health physicians are Board Certified or board eligible in Family Practice, and Internal Medicine.
Since its founding in 1977, Rural Health Care, Inc. (Aza Health) has grown to include locations in six contiguous counties in northern Florida: Alachua County, Clay County, Flagler County, Putnam County, St. Johns County and Volusia County.
Today Aza Health operates 11 full-time primary care centers offering a wide range of services, three full-time dental centers, and nine full-service pharmacies. In addition, with more than 250 employees, Aza Health is a major employer in the area.
Aza Health receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
Aza Health is governed by a volunteer Board of Governors that meets each month. Among their duties: defining and preserving the mission of the organization.
